Medical, Dental, Vision, and Prescription Insurance
Medical, Dental/Vision self-funded plans administered by Employee Benefit Management Services (EBMS). The plans include options for prescription drug program, preventative health checks, vision care, orthodontics and dental care, as well as other standard medical coverage. When using a network provider for medical services, employees enjoy a higher preferred provider benefit. Enrollment forms must be turned in within 30 days after hire date. Once enrollment forms are turned in, coverage goes into effect on the first day of the following month.

Flexible Spending Accounts
Pre-tax earnings may be set aside for eligible out-of-pocket health care and dependent care expenses. Coverage amounts of up to $1,500 for Medical Care or $5,000 for Dependent Care accounts are allowed each calendar year.

Life Insurance
Portneuf Medical Center provides for a benefit equal to one times annual salary up to $50,000 for staff employees or $90,000 for managers/directors. The benefit doubles for accidental death. Rates and testing requirements to be approved for additional supplemental coverage amounts are determined by age and tobacco use.

24-Hour Accidental Death & Dismemberment
This plan covers losses incurred as a direct result of accidental bodily injuries independent of all other causes and within 365 days from the date of such injuries and prior to the insured’s 70th birthday.

Long Term Disability
If employee becomes disabled and cannot perform the duties of their job, the benefit pays an amount equal to 60% of their basic monthly salary up to $6,000 per month after a 90-day disability waiting period. Coverage becomes effective after two years of employment.

401(k) Retirement Program
Employees have the option of choosing from several different investment funds managed by Prudential. Employees may contribute up to 60% of annual compensation, with an employer matching of up to 3% beginning after one year of employment in an eligible classification. After 2 years of employment in an eligible classification, an additional discretionary contribution may be made by Portneuf.

Paid Time Off
Depending on classification, employees accrue time to be used for vacation, holidays, short-term illness, and injury. A maximum accrual of 320 hours is allowed (100 hours for 7 on/7 off). Employees earn an extra pro-rated amount of up to 40 additional hours of PTO after every 5-year anniversary date. Accrual rates increase after 5- and 10-year increments of service.

For 0-5 years of service, employees accrue benefits as follows:
Exempt/Full-time: 7.076 hrs per pay period (includes holidays);
Part-time: 0.038 hrs per hour worked;
7 on / 7 off: 1.538 hrs per pay period;
Managers/Directors: 8.32 hrs per pay period.


Long Term Sick
Hours accrue at a rate of 2.15 hrs per pay period. A waiting period of 16 hours must be satisfied before accessing. Maximum accrual of 480 hours is allowed. Upon resignation, a percentage cash-out may be available.

Tuition Reimbursement
Employees may be eligible for up to $2000 per year for eligible tuition expenses. Reimbursement is made at 70% for courses in which a grade of “C” or better is earned, or for a “Pass” in a Pass/Fail course.

Continuing Education
Employees may be eligible to attend off-site seminars, workshops, and conferences and/or attend a variety of courses offered through the hospital’s Education Department, such as BLS, ACLS, PALS, NRP, TNCC, STABLE, Charge Nurse, Preceptor, and EKG courses.

Employee Assistance Program
Professional counseling service available for employees and their dependents. No charge for up to 5 visits per incident.

Employee Health & Wellness Program
Employees are encouraged to set self-improvement and wellness goals in exchange for rewards and incentives. Employees are also eligible for discounts and special promotions at several fitness centers in the area.

Shift & Pay Differentials
Employees working evenings, nights, weekends, and on-call receive additional pay on top of their base rate of pay.

Child Care Center – Kids Korner
Early childhood development programs in a caring atmosphere. Kids Korner is accredited by the National Association for the Education of Young Children (NAEYC) and accepts children between the ages of 6-weeks and 5 years.
