Listening to our patients and staff is essential to continuously improving how we take care of
people at Portneuf Medical Center. You may receive a questionnaire in the mail following
your hospital stay. The survey will ask for your opinion and advice about the care you
received during your stay. Your response will help us as we strive to maintain our standard
of excellence in patient care. No postage is needed to return the questionnaire.In addition, if you have a concern about the care you received, or would like to let us know about a positive experience with a Portneuf Medical Center staff or care area, we suggest that you talk directly with your caregiver or his/her supervisor.
