Advance Directives
An Advance Directive health form can help you give instructions about your medical treatment (Living Will), name someone else to make health care decisions for you (Durable Power of Attorney for Health Care), express your wishes about organ donation and designate a primary care physician.

Living Will
A living will is a legal document, which allows you to choose whether you will receive artificial life-sustaining procedures or be allowed to die naturally, in the event that you have a terminal medical condition or you are in a persistent vegetative state.

Durable Power of Attorney for Health Care
Durable power of attorney for health care is a legal document that allows an individual to appoint an agent to make all decisions regarding health care, including choices regarding health care providers, medical treatment, and, in the later stages of the disease, end-of-life decisions.
