Medical Center General Admissions Information

The purpose of the admission process is to obtain information about you in order to provide appropriate care during your hospital stay. You will also be given information to help make your stay easier, and you will be given an opportunity to ask questions about your hospital stay and how billing for services will be managed.

Your First Visit

When you are a new patient at Portneuf, we will require some detailed information prior to your appointment. Please come 10-15 minutes before your scheduled appointment. Medicare patients can expect to complete several forms healthcare providers must obtain on behalf of Medicare, so you may want to allow additional time.

Insurance

If you have insurance, please bring your insurance card(s) when you come to your appointment. If your insurance requires a co-payment, we will collect that at the time of service. If you are paying for your services yourself, we require payment at the time of service. If you are insured, but do not bring your insurance card, your account will be set-up to bill services to you until we receive a copy of your insurance card.

Privacy

Information provided during the admissions process is confidential and may not be released without your authorization. If you do not wish to have information released about your condition or presence at the hospital, please communicate those instructions to Admissions personnel. This means that information or even acknowledgment that you are in the hospital will not be released to anyone including family and friends.
